Hi, I've been making Delias chocolate fudge cake for several years now.
Have noticed last few times icing has not thickened /set as well.
Any ideas where I may be going wrong.

If you haven’t already, perhaps try…
Use 70% cocoa solids dark chocolate
Use block butter that is soft not butter that is spreadable
Simmer the evaporated milk and sugar mix at a slightly higher temperature so that there are some bubbles gently breaking on the surface (or simmer on the low heat for a few more minutes).
